Online services

You can use online services at the surgery to book an appointment, order a repeat prescription, get a sick certificate, get test results, ask about a non-urgent medical problem and view your medical record.

Which online service to use?

There are different online services for different tasks.

Systmonline

Use your Systmonline account (on the Systmonline website) to:

  • Book or cancel a routine appointment (call the surgery for an urgent appointment)
  • Order a repeat prescription
  • Get your test results
  • View your medical record

Use your NHS account (on the NHS website or in the NHS app) to:

Use your Patchs account on the Patchs website to:

  • Ask a question
  • Complete an online form to ask about a non-urgent medical problem

Registering for online services

To register for online services, so you can book and cancel appointments, order medication and get test results please ask one of our receptionists who will be able to help you.

To be able to view your medical record using online services, please download and complete this form and bring it into practice.

You will need to bring photo ID and proof of your address to the surgery, details of acceptable documents are included on the form.

Being able to see your record online might help you to manage your medical conditions. It also means that you can even access it from anywhere in the world should you require medical treatment on holiday.

Registering for someone else on their behalf

You can register for online access to someone else’s record, which is called proxy access, if you are a carer or relative using the same form as above.

The person who is being given proxy access will need to bring photo ID and proof of address to the surgery, details of acceptable documents are included on the form.

Parents of children under the age of 12 will be given access to online services for the child’s record without completion of this form.

Even if you are a long-standing patient at the practice and our team know you well or if you have recently registered and provided some of this identify evidence we still require it to accompany these applications – the practice team have to follow these strict guidance to comply with the instructions laid down by the Department of Health.

More about Patchs

Use an online form on Patchs to tell us about a medical problem or ask an admin question.

Patchs should not be used for any medical problem that you think is an emergency, including a worsening infection.

Patchs should not be used for repeat prescription requests. Use Systmonline instead.
