Making a Complaint

Every patient has the right to make a complaint about the treatment or care they have received at Undercliffe Surgery.

We understand that we may not always get everything right and, by telling us about the problem you have encountered, we will be able to improve our services and patient experience.

Most complaints can be resolved directly with the practice. Please speak to a member of staff if you have a complaint; our staff are trained to handle complaints.  Alternatively, ask to speak to the Reception Manager or Practice Manager. You may wish to complain in writing so we have collated this form to help you should you wish. Please return the form to the surgery.

If you are complaining on behalf of some else, please use this form .

Please remember that our staff are following instructions and procedures specified by the doctors.  They will serve you best if treated with respect and courtesy.

If you feel uncomfortable raising your concerns or complaint directly with us or if you believe this is not appropriate, you can raise your complaint with the ICB who commission and pay for the NHS services you use at:
