Access to Medical Records

ONLINE MEDICAL RECORD

The practice is registered and complies with the Data Protection Act 1998 and General Data Protection Regulations.

To request access to be able to view your online medical record please download and complete this form and bring it into the practice.

You will need to bring photo ID and proof of your address to the surgery, details of acceptable documents are included on the form.

If you wish to have access to your dependants online medical record and the dependant is aged 13 or over they must complete this form before we can process this request.

SUBJECT ACCESS REQUESTS 

In accordance with Article 15 of the UK GDPR, individuals have the right to access their data and any supplementary information held by Undercliffe Surgery. This is called a Subject Access Request (SAR). SAR’s are predominantly used for access to, and the provision of, copies of medical records.

Who can make an Access Request?

Requesters must be:

  • The data subject OR
  • Have the written permission of the data subject OR
  • Have legal responsibility for managing the subject’s affairs to access personal information about that person

It is the requester’s responsibility to satisfy this organisation of their legal authority to act on behalf of the data subject. The organisation must be satisfied of the identity of the requester before they can provide any personal information.

Application

Individuals wishing to exercise their right of access should:

  • Make a written application to the Practice holding the records
  • Provide such further information as the Practice may require to sufficiently identify the individual

The Practice requires completion of this form for all Subject Access Requests.  As evidence of identity is required with submission of this form please see the list of acceptable documents detailed on the form.

We may send the data to you via email dependant on the size of the file. Alternatively the information will be stored on a password protected disc which will require collection from the Practice.
