Access to On-Line Medical Records

Access to On-Line Services

If you wish to, you can now use the internet to book and cancel appointments with a GP, Advanced Practitioner, or to have blood tests, request repeat prescriptions for medications you take regularly or send a message to the surgery (not urgent). You can also view your summary care medical record online. You can also still use the telephone or call in to the surgery for any of these services as well (however, we do not take requests for repeat medication over the phone). It’s your choice.

Being able to see your record online might help you to manage your medical conditions. It also means that you can even access it from anywhere in the world should you require medical treatment on holiday.  Access to the detailed coded electronic medical record is also now available; this access will be granted on a patient by patient basis on receipt of the completed necessary form(s).

Please see links below to the necessary forms that must be completed and brought to the surgery in person with the necessary acceptable identity evidence. Even if you are a long-standing patient at the practice and our team know you well or if you have recently registered and provided some of this identify evidence we still require it to accompany these applications – the practice team have to follow these strict guidance to comply with the instructions laid down by the Department of Health.

– Please complete this form if you wish to be given access to your Detailed Coded Record and bring it into the surgery along with the acceptable identity evidence.

– This form is to be used if you wish to grant access to a spouse or carer and for children.

You should be aware that in certain circumstances your right to see some details in your health records may be limited in your own interest or for other reasons.
